Note: -
1. This permit will be initialed by user department, but will be authorized by the department head in whose dept.
The work will be carried out
2. This permit is valid only on the date of issue and the tirne specified. Fresh permit to be taken everyday.
3. Incase any work of open flame is to be done in the area ‘HOT WORK PERMIT’ must accompany this permit
4. This permit should be in possession of the person carrying out the work and he should return the same to his
supervisor after the job is over.
5. It must be made sure that the person on the job fully understands the prescribed conditions.
6. This permit does not relieve the permit user from his responsibility to safety.
7. Site conditions must be personally checked by permit issuer and user dept. personnel.
8. Ensure that a safety observer is kept out side throughout the work .In the event of discontinuation of work the permit
should be revalidated by SAFETY OFFICER
9. This permit is to be issued during daytime only.
10. Authorized signature means unit head,
11. Electrical isolation to be carried out by Electrical dept before starting the job.
12. All electrical connections must be checked by electrical dept.
13. No alteration should be done in permit.
14. In case of emergency, follow instructions of shift officer.
15. For same day, Oxygen content to be checked for re-entry by safety.
16. Hand lamps if required to be of 24 Volts source.
