Hazardous / flammable / explosive material transfers disconnected within 21 m of hot work?
Lockout / tagout of electrical, mechanical, chemical, blanking, cap piping implemented?
Vessels, equipment drained, purged, ventilated, and cleaned?
Inert gas blanket required?
Building/area air currents and outdoor wind direction known / monitored?
Hazardous chemical spill kit location equipment and remedial plan available?

APPROVALS AND AUTHORIZATIONS

This permit is valid as long as work conditions existing at the time of issuance continue.
It expires on any change of condition that adversely affects safety of the work area while work is in progress.
