Work Activity:

Event Description: On the above-mentioned date, time & location. HEXATECH electrical team working in above
mention area. At the same time HBK civil team also engage in fixing Tabuk blocks in ground floor suddenly one Tabuk
is fall from ground floor opening through the basement 01 zone b area. Tabuk block weight around 40Kg from the
height 5m. Its poor negligence & lack of supervision from the civil team the Civil team (HBK) co – worker & there is no
barricade & signboards on the work area.

Immediate Action(s) Taken

 Informed HBK HSE Manager
 Barricaded the Near miss area with warning tapes by HEXATECH
 Advice the concern foreman (mason)

Corrective / Preventive Action(s) Plan

 Standdown meeting conducted with HEXATECH operatives along with supervisor
 Standby person must be in basements while working in ground floor opening area.
 If any work on floor opening need to check the areas & make sure no one is working beyond the opening point
