- Evident that monitoring not fully compliance with onboard emergency readiness requirement :
+ SOPEP lack chemical
+ FRC only has 1 painter at bow
+ Portable Multi Gas Detector not onboard

Nguyên trực tiếp/ Immediate cause:

- Officer in charge did not control SOPEP properly so that Crew use ODS chemical for other purpose
- One painter line of FRC was missed when it had received from maintenance service company on
24/11/2017
- Crew and officer did check FRC carefully when received it
- Other vessel borrow ship’s Gas detector for doing in enclose space so that it was not available on that
time

- Make an order for ODS chemical with the quantity as required in SOPEP immediately
- Make one more painter line for FRC so that two painter line always ready for and aft for
launching
- Require the vessel borrow Gas detector to return it before vessel departure
Hành động ngăn ngừa tái diễn/ Action taken to avoid re-occurrence:

- Officer should manage emergency readiness equipment with quantity and their certificate always
onboard as the required of emergency plan.
- Officer has to carry out check and maintain emergency equipment properly follow PMS
