Secure the Request for Rescheduling of Exam Form from the Academic Head. 2. Fill out the form and identify the course(s) that you want to reschedule. 3. Consult your Course Instructor about the exam to be rescheduled. 4. Submit the accomplished form to him/her at least three (3) days before the 1st day of the examination period. 5. Get the approved form from your Course Instructor on the day the form was submitted for approval.
The exam shall be rescheduled on the same exam date.
Do not forget to bring the approved form during the rescheduled exam. Failure to take the exam on the given schedule shall result to a failing score.
