Clinical Diagnosis: Tenderness in right iliac fossa, low grade fever and loose stools
CBC -Hb-12.2 g/dl, TLC -91000/ul, Platelet count -3.03 lakh/ ul

Peripheral Blood Smear :

RBCs - Mild anisopoikilocytosis with admixture of normocytic normochromic cells, microcytic hypochromic cells with few polychromatophilic
RBCs.

WBCs - Hyperleukocytosis with shift to left upto blast.( 5%)

Platelets - Adequate on smear.

No hemoparasite seen. R/C - 0.3%


Impression - S/O Chronic myeloproliferative disorder.

Cellularity –Markedly Hypercellular (~ 95%) M:E ratio –20 :1


Erythropoiesis - Marked decrease in erythropoeisis with with few early normoblasts seen.
Myelopoiesis – Markedly increased in granulocytes and their precursors and maturation upto neutrophils is seen. Blasts are approximately 5%

Megakaryopoiesis -There are presence of few micromegakaryocytes with dysplastic forms exhibiting hypolobated nuclei.

There is no granuloma/ parasites seen.


Perls stain +1

Impression: Hypercellular marrow with marked myeloid hyperplasia and dysplastic micromegakarocytes favoring Myelodysplastic/
myeloproliferative neoplasm.
Advise : 1.Cytogenetics for further typing.
2. Kindly corelate with biopsy report.
