Bring ONE of the following: Bring TWO documents with your first and last
Current, valid Washington driver license or ID card name and current WA residential address
(not a PO Box):
Valid out-of-state driver license or ID card
Your WA License/ID or spouse’s WA DL/ID (with
Government (federal, state, county, and municipal)
marriage certificate or unexpired military ID)
employee ID card
WA vehicle, vessel, utility trailer title or registration
Valid U.S. military ID card
Utility (electric/gas/water/sewer/garbage)
Valid U.S. passport or passport card
(printout OK)
U.S. Certificate of Citizenship/Naturalization
Telephone/cable/cell phone/internet bill (printout OK)
Parent/Guardian for applicants under 18 (see Minors)
Auto, home, or renter’s insurance policy or bill
Minors USPS change of address letter
Personal mail, postmarked
If you are under 18 years old, your parent/guardian
must: Bank or credit card document (printout OK)
1. Go with you to the licensing office. This is a partial list. For a complete list see:

2. Sign the application.
3. Bring proof of their own identity from the list above. Name change, if applicable
4. Bring proof of relationship to you:
If the name on your identity and citizenship
Certified birth certificate documents is different, bring as many of the
Certified adoption document following documents as necessary that connect the
Certified court order naming the guardian and two names:
specifically authorizing EDL/EID Court order (filed and certified) showing name change
You will need all the documents required for Divorce decree (filed and certified by the court)
proof of citizenship, identity, SSN, and residence. showing the new name or authorizing a name change
You may bring residency documents in your Marriage certificate (filed and certified). Church or
parent or guardian’s name. wedding chapel forms aren’t acceptable.
