Introduction
LTIMindtree (the “Company” recognizes the value of
benefits to employees and their families. The Company
(“LTIMindtree”) supports employees by offering a
comprehensive and competitive benefits program.
Eligibility for each benefit is governed by the terms and
conditions of the applicable plan. For more information
regarding benefit -plans, please refer to the applicable
Summary Plan Descriptions (SPDs), which can be
obtained from the Human Resources Department. To the
extent the information provided here conflicts with the
SPD or full plan document, the full plan document will
control.

LTIMindtree provides employees with four
(4) options for medical plans in addition to
two (2) dental and two (2) vision plans.

Medical Coverage
LTIMindtree offers health insurance through Cigna to employees and their families (spouse and unmarried children
under the age of 26). Health Insurance Coverage is effective from your date of hire in USA provided you enroll within
31 days of joining. Enrollment can also be done during Qualified Life Event (QLE) and during open enrollment.

Prescription Drug (Rx) Coverage
If you are enrolled in one of LTIMindtree medical insurance plans, you are automatically provided with prescription
drug benefits through CIGNA Express Scripts. This program also provides a Home Delivery pharmacy. Through this
coverage, you also have an option to choose from ‘CIGNA 90 Now CVS’ and ‘CIGNA 90 Now’ program.

Dental Coverage
LTIMindtree offers dental insurance through the CIGNA DPPO and CIGNA EPO plan. Through Dental PPO, you may
choose whether you use a participating dentist or not. The network is extensive, and the benefits are similar in- or out-
of-network.; however, your benefit level is higher if you use in-network providers. Preventive care is covered at 100%
under both in-or out-of-network (subject to plan limits). Through EPO, there is only in-network coverage.

Vision Coverage
LTIMindtree offers two (2) vision plans – Core Vision and Enhanced Vision plans, through CIGNA vision serviced by
EyeMed. The vision benefit is designed to provide routine preventive care such as eye exams, eyewear and other
vision services. This benefit utilizes the CIGNA Vision Network, and there are no claim forms to complete
when you visit an in-network provider.

Program
The Company provides employees with an Employee Assistance
Program (“EAP”) through Cigna at no cost, which is available to assist
employees and their immediate family members with a variety of
personal and family issues that may impact life at home or at work. The
EAP provides 24/7 support through a toll free # 1877- 622-4327 or at
mycigna.com. The service is free, confidential and offers consultations
on relocations, emotional health, legal matters, financial matters, home,
life referrals, suicide awareness and prevention, critical incident
response (natural calamity) etc. The EAP also offers behavioral
awareness sessions and wellness seminars.

Health Savings Account (HSA)
LTIMindtree offers employees a Health Savings Account (“HSA”) through HSA Bank Account for those who enroll in the
HSA Medical plan. The HSA enables employees to take advantage of tax savings and set aside funds for eligible
healthcare expenses. The annual maximum contribution amount under HSA account is subject to IRS limits..

401(k) Retirement Saving Program

The Company recognizes the importance of saving for retirement and offers eligible employees a 401(k) plan – both
Traditional (pre-tax) 401(k) and Roth 401(k) option. Participation in the plan is purely voluntary. You may enroll in the
Company’s 401(k) plan in the month following your date of joining in USA.

To enhance our employees’ ability to save for retirement, the Company offers an employer matching contribution.
These percentages also apply to catch up contributions. Employees are eligible to receive the employer match after
completion of three months on US payroll. The match starts in the month following eligibility.

Full Purpose Healthcare Flexible Spending Account


A Full Purpose Health Care FSA is available if you are enrolled only in Core PPO, LocalPlus and HRA Medical Plan. It can
reimburse you with your pre-tax dollars for eligible medical, dental and vision expenses, up to the amount you
contribute for the plan year. The annual maximum contribution to the Healthcare FSA is subject to IRS limits.

Limited Purpose Healthcare FSA


Limited Purpose FSA – If you intend to participate in the HSA medical plan, you can enroll in the Limited Purpose FSA
to cover your dental and vision expenses not covered under your HSA medical plan (which is used for medical and
prescription drug expenses only). The annual maximum contribution to the Limited Purpose FSA is subject to IRS limits.

Dependent Care Flexible Spending Account


LTIMindtree offers employees the opportunity to set aside up to $5,000 per plan year for individuals or married
couples filing jointly, or $2,500 for a married person filing separately for eligible dependent care expenses.

Commuter Benefits (Transportation and Parking Expenses)


LTIMindtree provides commuter benefits through HealthEquity (WageWorks) to allow employees to pay for
commuting expenses (train/bus/subway/parking/meter garages etc.) through pre-tax payroll deductions subject to the
annual IRS limits.

LTIMindtree believes in encouraging its employees to spend quality time for their own wellbeing in order to
achieve a healthy work/life balance. The Company provides the following leave benefits to eligible employees

• Sick Leaves Utilization & Carryforward guidelines will be based on Work locations

• Advanced Paid Time Off (APTO): upto 24 hours per year

• Bereavement Leave: upto 16 hours of paid leave per year

• Parental Bonding Leave: upto 24 hours of paid leave per each instance.

• Jury Duty – 1 day of paid leave per year

• Time off to Vote 2 hours

• 10 Declared holidays (Only for LTIMindtree offices)

Basic Life and AD&D Insurance
All employees are covered by LTIMindtree’s Basic Life and AD&D insurance benefits equal to 1.5 X annual base
salary (minimum of $250,000, up to a maximum of $1 million). LTIMindtree pays 100% of the cost for basic life
insurance and AD&D coverages. Dependents (spouse and children) are not covered under this benefit.

Voluntary (Supplemental) Life insurance


Supplemental Life insurance is an additional layer of coverage employees may purchase to help financially
protect their families. Employees can purchase supplemental life coverage for themselves, a spouse and their
children at the time of joining or during open enrollment only.

Benefits are as follows


Employee: Employees can elect the lesser of 8 times their annual base weekly salary or $1,000,000 in $10,000
increments. The minimum election is $10,000.

Spouse: Employees can elect up to the lesser of 50% of the amount the employee elected or $250,000 in
$5,000 increments. The minimum election is $5,000.
Child: Employees can elect up to $15,000 in $5,000 increments.

Short & Long-Term Disability (STD & LTD)


Disability insurance is coverage that provides you with income protection, should you lose time on the job due
to an injury or illness. With disability coverage, partial replacement of lost income is paid to you. LTIMindtree
offers short-term and long-term disability coverage at no cost to you. Short-term disability pays 66.67% of an
employee’s gross weekly earnings for up to $1000 per week for13 weeks in the event of a covered sickness or
injury. If an employee becomes totally disabled and cannot work for an extended period, long-term disability
benefits pay 60% of the employee’s gross monthly pre- disability earnings subject to the policy limits

LTIMindtree offers a wide range of Voluntary benefits related to health, legal, pets and identity theft. These Voluntary
benefits are supplemental to your core benefits and offered to meet your family needs. You can enroll in Voluntary
benefits at the time of joining or during Open enrollment through post-tax payroll deductions.

Voluntary Health Benefits


LTIMindtree offers the following additional voluntary health benefits - Accidental Injury Insurance, Critical Illness
Insurance and Hospital Care Indemnity Insurance through Cigna. Employees contribute to these plans through post-
tax payroll deductions and these voluntary plans pay directly to you based on coverages, not the doctor or hospital in
case of any claims. By enrolling in group coverage through LTIMindtree, you will receive coverage at a discounted rate.

Accident Insurance
CIGNA Accident insurance is designed to help defray the out-of-pocket costs associated with an accidental injury.

Critical Illness
CIGNA Critical Illness insurance is designed to help defray the costs associated with a heart attack, stroke, or other
covered serious illness as defined in the policy.

Hospital Indemnity
CIGNA Hospital Indemnity Insurance provides a benefit if you or a covered dependent are admitted or confined to a
hospital as a result of an accident or sickness.

Wellness Screening Benefits


Included with all three Cigna voluntary benefit plans (Accident, Critical Illness and Hospital Care Indemnity) is a
Wellness Screening Benefit. This benefit pays each covered individual (including spouses and children) $50 per year
for wellness visits and tests.

MetLife Legal Services Plan


LTIMindtree offers Voluntary MetLife Legal Services Plan which provides legal representation and covered legal
services through convenient payroll deductions for a wide range of personal legal matters.
Employees contribute to these plans through post-tax payroll deductions.

MetLife Pet Insurance


With LTIMindtree’s Voluntary Pet Insurance through MetLife, you can protect your furry family members from
unexpected visit to the vet by obtaining a group discount.

Allstate ID Theft Protection


LTIMindtree’ s Identity Theft Protection offered through Allstate monitors your accounts and provides protection in
the event your identity is compromised. Enrollment in Allstate’s Identity Protection plan gives you covered access to
monitoring that catches fraud at its earliest with proactive dark web, tri-bureau credit, financial transaction, and social
media account monitoring.

SoFi Loan Refinance


LTIMindtree has partnered with SoFi at Work to offer products to help reach your financial goals. Through this
program, employees are eligible to receive a better refinancing rate for children’s education, mortgages and personal
