LETTER OF AUTHORIZATION FOR CERTIFICATE COLLECTION
I, Ezeani Christabelle Chisom a graduate of the Department of Computer Science with reg
number 2017/242454 at this moment, authorize Mr Izuchukwu Uzo an academic staff of our
institution with staff number SS.12046 from the Department of Computer Science to collect
my degree certificate on my behalf. I cannot collect the certificate in person due to pressing
reasons that are beyond my control.
My certificate is a basic requirement for an urgent official purpose with a strict deadline that
ends long before the conclusion of my service. I am currently fulfilling my National Youth
Service Corps (NYSC) obligations in a location that is considerably very far from Nsukka
and NYSC regulation restricts my movements to the state of service. I cannot travel outside
the state for the duration of the service program.
I kindly request that the necessary arrangements be made to release my certificate to Mr
