THREE-PHASE BLOOD COMPATIBILITY TESTING
Background:
Compatibility test or cross matching is performed subsequent to the ABO grouping and
Rh typing of the recipient’s and donor’s blood. It is the final criterion as to the suitability
of a particular donor blood for a particular recipient.
The recipient’s blood is obtained fresh while the donor’s blood is obtained from the pilot
tube. CPDA-1 anticoagulant donor’s blood should not be more than 35 days and
constantly stored at 4°C.
Principle:
Serum of the recipient is tested against the red cells of the donor under different
conditions in order to establish their cross matching or non-agglutination. Agglutination
in any of the conditions indicates the presence of incompatible antibody in patient,
natural or immune.
Procedure:
1. Label test tubes properly.
2. Add 2 drops of patient’s serum/plasma
3. Add 1 drop of 3-5% donor red cell
4. Centrifuge for 1 minute
5. Read and Record the result of the IMMEDIATE SPIN (IS)
6. Add 2 drops of LISS
7. Mix and incubate for 15 minutes at 37 degrees Celsius (dry bath)
8. After incubation, centrifuge for a minute
9. Read and record result – THERMO PHASE (37 degrees Celsius)
10. Wash 3-4x with large amount of NSS
11. After the last washing, blot the tube in a gauze pad/ tissue paper to absorb excess
saline
12. Add 2 drops AHG reagent
Interpretation Remarks:
No hemolysis/ No agglutination/ COMPATIBLE
With hemolysis/ with agglutination/ INCOMPATIBLE
