PRELIMINARY INVESTIGATION REPORT


PT-6 AC SER NO 6104 ON 22 OCT 09 AT ARULIA AIR FIELD, BOGRA

Introduction

1. On 22 OCT 09, PT-6 ac ser no 6104 flown by Sqn Ldr AAMM Shamsuzzahan (BD/8638), GD(P), Instr plt of FIS, and
Sqn ldr Jamil Uddin Ahammed (BD/8732), GD(P), student plt of FIS, BAF made a crash landing approx. 100 ft short of
threshold RW 30 in Arulia airfield, Bogra. After completion of their FHT msn, plt returned to base and carried out cct &
ldg.

Description of the Incident

2. On 22 OCT 09, PT-6 ac ser no 6104 flown by Sqn Ldr AAMM Shamsuzzahan (BD/8638), GD(P), Instr plt of FIS, and Sqn
ldr Jamil Uddin Ahammed (BD/8732), GD(P), student plt of FIS, BAF were detailed to fly FHT-2 msn in PT-6 ac Ser no-
6104. They took off at 1044 hrs from RW 30. After completion of msn they returned to base and carried out cct & ldg.
Finally they joined bad wx downwind for full stop ldg. At the end of the bad wx downwind plt gave call for PFL.
Thereafter on final app they failed to make the RW and made a crash ldg apprx 100 ft short of threshold RW 30. Plts
came out from the crashed ac with the help of BAF pers present there and apparently it seems that they didn’t have any
visible injury.

SAR Effort

3. SAR was provided by Bell-206 hel. The SAR cdr along with president BOI and BFSO took off from MTR at 1233 hrs. By
1830 hrs of that day, the wreckage was discovered.

Preliminary Findings

4. The undersigned carr out the preliminary investigation and found the fol:

a. On 22 Oct 09, by Sqn Ldr AAMM Shamsuzzahan and Sqn ldr Jamil Uddin Ahammed were planned to fly FHT-2 msn.

b. Both the plts were medically fit and had their snacks in sqn.

c. Normal/emergency session for the day was conducted.

d. ‘Authorization Book’ was filled and the flt was duly auth.

e. F-781 was duly filled by plt.

f. During takeoff, at about 1044 hrs, the airfield state of FIS, BAF was “Yellow” & Met warning was “Yellow” for poor
visibility. No significant weather activity was observed over and around the air fd. But visibility was decreasing time to
time.

g. During the crash, ac was comparatively low in ht and not aligned with the RW center line.

h. Apparently, it appeared that on short final while trying to align with the center line rt wg of the ac hit the grd apprx
100 ft before RW 30 and due to the impact rt wg has broken and ac toppled short of threshold RW 30.

j. Plts came out from the damaged ac by the help of some BAF pers present there without any visible injury.
Thereafter, they were sent to CMH, Bogra by ambulance.

k. The ac was damaged severely.

l. No other persons were injured and no damage was done to the civilian property.

m. Photographs of the salvaged wreckage were taken and handed over to the member of BOI.

n. The dmd for the transcript of R/T conversation between plt and ATC twr/ mobile/ supervisor of FIS was placed to
ATC twr.

Analysis of the Wreckage

5. As general rule, it can be borne in mind that, for a slow moving ac, as it crash landed in a solid object. No of wreckage
salvaged was less in number and found out easily within the crash site.

a. Ac Attitude during Impact. From the crashed ac, with the cut marks and other signs it is clear that the attitude was
low than required and the ac was far from the RW threshold.

b. Impact Angle. The impact angle of the ac was probably 20 – 30 degree. By seeing the crashed photos we can
determine that.

c. Determination of Speed during Impact. The ac was in downwind and they were practicing bad wx cct. With the

investigation from the 1st pilot, 2nd pilot, ATC offr and mobile offr the speed during impact was 150 kph.

d. Leftover Wreckage of the AC. As the ac was crash landed in the short of RW threshold and in front of BAF pers, so
all the wreckages were found and rescued securely.

6. The following may be the cause of the accident:

a. IP had very less reaction to control the flight when over correction of the controls applied by the student pilot in
the final leg.

b. System failure of the ac.

c. Reduction of visibility followed by leading the plt to take a wrong decision.

d. IP did not took over the control while seeing their position.

e. Mobile offr did not alert them for the low approach earlier.

f. Airfield weather information was not updated by the ATC pers.

7. The exact causes of the incident could not be ascertained yet. A BOI has been ordered to find out the actual/probable
cause of the incident.
