DRIVER’S AFFIDAVIT
I, ROLAND D. SELIDIO, of legal age, Filipino, married, and presently residing at MVC MT.
Nebo, Valencia City, Bukidnon, after having been duly sworn to in accordance with law do hereby
depose and say;

A. That I am a holder of a Professional Driver’s license with No. K03-03-067783 issued by the
Land Transportation Office (LTO) Valencia City, Bukidnon which will expire on September 21,
2032;

1. That I was driving my TOYOTA PICK-UP HILUX 2.8 4X4 with Plate No. LAB4924, Engine
No. 1GD4375244, Chassis No. MR0HA8CD0H1088973 with OR No. 294743470 and OR
No. 1012000000062660;

2. That on March 15, 2024, on or about 1:30 in the afternoon I was traveling from Mountain
View College (MVC) Valencia City, Bukidnon bound to Malingon Falls inside MVC
Compound;

3. I didn’t notice the branch of a tree that is blocking the way causing visible damage to the
Front Fender RH, Front Door RH, Rear Door Rh, and Rear Trunk RH of the above-
mentioned unit and also;

B. On March 18, 2024, at or about 12:50 midnight I was traveling from the east heading towards
the west direction going to Mt. Nebo, Valencia City while Kawasaki CT100 was traveling in the
opposite line from west to east direction. The incident happened when Kawasaki CT100
encroached the line of my unit Toyota Pick-up Hilux;

1. As a result, both vehicles involved incurred damages on different parts and accessories
that only expert mechanics can determine.

2. That I have voluntarily executed the above statements in support of my insurance claims.
