DRIVER’S AFFIDAVIT

I, CARLITO M. SARABIA, of legal age, Filipino, married with residence and postal address at San
Miguel, Digos City, Davao del Sur, after having been duly sworn to in accordance with law, hereby
deposes and says that:

1. I am the driver of Mitsubishi Strada GLS, color Virgil Gray with plate no. LAA 9360
registered to Mt.Kitanglad Agri-Ventures Inc;

2. On March 22, 2023, on or about 8:30 a.m., I was travelling at Lantapan Provincial Road
going to MKAVI, and upon reaching at Purok 2, Alanib, Lantapan, Bukidnon, a flying stone
due to an unidentified dump truck coming from the opposite direction accidentally hit the
vehicle’s windshield, particularly on the upper portion of the driver’s side, thus, resulting to
damage on the vehicle.

3. The damage to the vehicle occurred purely by accident and without any intention.

4. I execute this affidavit to arrest the truth and veracity of the foregoing facts and
statement and for whatever legal purpose it may serve.
