This is to acknowledge the receipt of the endorsement letter of the trainee referred
underneath relative to his request for a work immersion.
Strand: STEM
In view of the requirement of the said training, it is directed to consider his request, subject
however to the condition that the above-mentioned trainee shall comply accordingly with
our company OJT program policy.

Note: All personal information disclosed shall be used solely for legitimate purposes,
specifically for the on-the-job training of the above-named student and shall be processed
by authorized personnel in accordance with the Data Privacy Policy of the Institute and the
Company.
This is to grant the request of the Immersion Trainee referred underneath relative to his/her
request for the virtual immersion program in lieu of the physical company immersion for the
period covering the 3rd Quarter, 2nd Semester of A.Y. 2023-2024.
In view of the requirement of the said virtual immersion program, it is directed to consider
his/her request, subject however to the condition that he/she shall comply accordingly to the
following terms and conditions:
a. That the total no. of hours required for the trainee to complete his/her virtual
immersion program is equivalent to 80 training hours.
b. That the trainee shall abide by the Host Company’s rules and regulations and comply
with those imposed for the virtual immersion program, otherwise he/she shall be
excluded from further participation.
c. That the trainee may have access to information which may be of a confidential nature
and proprietary to the Host Company, for which he/she may be required to execute a
confidentiality and non-disclosure agreement as a pre-requisite for his/her my
participation in the virtual immersion program;
d. That the trainee will deliver the project/projects assigned to him/her during the virtual
immersion program regardless of if he/she were issued a certificate of completion or any
of its equivalent and that Mapúa shall not be made liable in the event he/she will not be
able to deliver such;
e. That the Host Company reserves the right to discontinue the immersion training on
reasonable grounds upon written notice to Mapúa;
f. That the trainee allows the Host Company and Mapúa to use his/her personal
information which may be disclosed for legitimate purposes, specifically for monitoring
his/her participation in the virtual immersion program, and allow the processing of that
information only by authorized personnel in accordance with the Data Privacy Policy of
the Host Company and Mapúa.
NOTE:
The host company may revise the provisions stated herein subject to review and approval of Mapúa.

I do hereby voluntarily undergo an on-the-job training under the following terms and
conditions:
a. That the total no. of hours required of/for me to complete my work immersion program is 80
hours;
b. That I shall abide by the firm’s/company’s rules and regulations and comply with those imposed
for the work immersion program, otherwise I shall be excluded from further participation;
c. That in the course of my work immersion program, I may have access to information which may
be of a confidential nature and proprietary to the firm/company, for which I may be required to
execute a confidentiality and non-disclosure agreement as a pre-requisite to my participation in
the program;
d. That the time I will spend on the work immersion program will not and should not be
interpreted or construed as working hours and should be regarded as non-compensable,
provided that, the firm/company may as a unilateral act of liberty or generosity on their part,
provide me with meal, travel, transportation allowances, accommodations, etc.;
e. That I fully understand that notwithstanding the allowances enumerated in the preceding
section which I may receive, there exists no labor-management and/or employer-employee
relationship between me and the firm/corporation where I will undergo my training;
f. That I will deliver the project/projects assigned to me during the course of my training regardless
if I were issued a certificate of completion or any of its equivalent and that Mapúa shall not be
made liable in the event I will not be able to deliver such.
g. That I shall personally be made answerable for any and all liabilities for damage to property or
injury to third person, which may be occasioned by my intentional or negligent acts during the
course of my work immersion;
h. That I shall likewise hold the firm/company and Mapúa free and harmless from any and all
liability and responsibility for any sickness or injury to myself and third parties and damage to
property which I may sustain and/or may occur at any time during the training program period
including time spent in travelling to and from any and all premises and locations where I may be
required to go to as part of my training program;
i. That the firm/company reserves the right to discontinue my training on reasonable grounds
upon written notice to MAPUA. Additionally, in the event my training program is discontinued
for reasons attribute only to myself, I may be made to reimburse the firm/corporation for any/all
the allowances, stipends, etc., which I received from the firm/corporation during and prior to the
termination of my training program;
j. That in addition to my liability under section (e) and for the pre-termination of my training
program provided for under sections (g) and (h) hereof, I may be subjected further to disciplinary
action in accordance with the school’s student manual and/or be a ground for disqualification
from graduation.

k. That MAPÚA may likewise terminate my work immersion and withdraw my participation thereto
on reasonable grounds and after a written notice to the firm/corporation and to myself;
l. That I allow Mapua to use the personal information which I disclosed for legitimate purposes,
specifically for monitoring my participation in the work immersion program of the Senior High
School, and allow the processing of that information only by authorized personnel in accordance
with the Data Privacy Policy of the Institute.
m. That I have read this document and understand the risks and responsibilities associated with
participating in the internship program.
