AGREEMENT

a) I, the Parent / Guardian of the above-named student, hereby aware of the graduation
practice protocols and rules stipulated herewith.

b) I agree to direct my child to cooperate and conform to the legal arrangements of the
school and the personnel responsible for the aforementioned activity.

c) I also grant my permission for the organizers of the activity to exercise authority and
related consequence in my behalf.

d) In recognition to these realities and rules and in consideration to the opportunity given I
agree to release, waive and quitclaim the Maryknoll School of Lambajon, Inc.
administration, faculty, staff and organizers free from all liabilities and claims.

Graduation Practices’ Protocols and Rules

a. The student shall hereby attend graduation practices starting today March 9, 2020
and onwards.
b. The practices schedule shall be posted or informed after the end of every practices.
c. The students are not allowed to have 3 consecutive absences or at 3 least absences
without valid reasons, only for the following are considerable:
 Death of a mediate family
 Health risk and other untoward major incidents
d. Three (3) reported tardiness is equivalent to 1 absent.
e. If the students violate the entire agreement, the student / s is/ are hereby
recommended not to attend graduation ceremony effectively during violation.

ATTESTATION

I have read, and understood the information above. I hereby voluntarily affix my
signature signifying conformance to the foregoing.
