Agreement on Participation to the 20th Commencement Exercises,

Graduation Photo Package, and Use of Personal Information

To properly disseminate information regarding the participation of alumni from the Class of 2020 and 2021 to the
20th Commencement Exercises; availment of Graduation Photos; and the use of alumni’s personal information by
the Alumni Center; please be informed of the following:
1. All fees related to the 20th Commencement Exercises are included in the set of graduation-related fees.
By signing this form and by participating in the 20th Commencement Exercises, the alumni authorize
MCL to bill the charges for graduation-related fees.
2. Alumni who will participate in the 20th Commencement Exercises will not be included in the Blue &
Silver Yearbook and the annual graduation souvenir program.
3. Alumni who will participate in the graduation photoshoot authorize MCL to bill the charges for the
graduation photo package.
4. As part of the health and safety precautions to prevent the spread of COVID-19, there will be no hair
and make-up services provided. The alumni must come to the photoshoot in their preferred hair and
make-up; and attire.
5. By availing the Optional Basic Photo Package, the alumni expressly grant their consent for their photos
and personal data to be retrieved and used by MCL for the multimedia production relevant to the 20th
Commencement Exercises and other legitimate purposes. Photo package includes the digital
retouching of photos, and 1 pc. 5R, and 4 pcs. 2R printed photo in formal attire.
6. By availing the Optional Upgraded Photo Package, the alumni agree to pay the published rate, and
to receive the following:
- 10x12 graduation photo in toga; in 12x14 inches glass-to-glass frame
- 1 pc. 5R photo in creative shot; 1 pc. 5R photo in formal attire.
- 4 pcs. 2R photo in toga; 4 pcs. 2R photo in creative shot; 4 pcs. 2R photo in formal attire.
7. All contents and inclusions of the basic and upgraded photo packages is a deliverable of the outsourced
photography service provider that MCL will contract for the purpose.
8. Photos taken will also be used for the multimedia production relevant to the 20th Commencement
Exercises. Those who will not participate in the official photoshoot will still be included in the 20th
Commencement Exercises, but the picture to be used is the photo submitted during the Testimonial
Ceremony.
9. All relevant procedures are compliant of the Data Privacy Act of 2012. Hence, any alumna/alumnus who
availed the photo packages gives consent for the use of his/her photos, and the agreement on the
disposal of the data by MCL and its contracted provider.
10. Processing and delivery of photos will take between 60 to 90 working days from the date of photoshoot
with possible delays that may be caused by challenges on the completeness of data and photos to be
used and logistical considerations.
11. Contact details and other personal data given by the alumni during the photoshoot may be used by the
Alumni Center for the tracer study and other legitimate purposes. The alumni also hereby agree to have
these data shared to partner companies for career placement support initiatives.
In witness whereof, to signify acceptance of the foregoing, this document is signed on the date stated below. It
is also understood that any alumni may refuse signing but required to communicate his/her objection via email:

consolidate, block, erase or destruct the personal data they have submitted.

The user hereby affirms their right to be informed, object to processing, access and rectify, suspend or withdraw their
personal data, and be indemnified in case of damages, pursuant to the provisions of the Republic Act No. 10173 of
the Philippines, Data Privacy Act of 2012 and its corresponding Implementing Rules and Regulations.

Furthermore, the affiant whose signature appears below (with the parent/guardian for persons of minor age) agree
that the submission of his/her photograph is voluntary; and that the use of the same for marketing and promotional
purposes is warranted and acknowledged to be legitimate and lawful.
