SUBJECT : Spot Report: Poisoning Incident Resulted Chavez Jules

DATE : April 07, 2024

1. This has referenced to Memorandum Spot Report Re: Poisoning Incident resulted to
the death of Chavez Jules dated April 02, 2024.

2. At about 9:30pm of March 29, 2024 at Saint Josapin Hotel Talaba 1 Emilio Aguinaldo
Hwy/ Longos Flyover Intersection Bacoor, Cavite

2. The victim, Chavez Jules, legal age, popular known by his alias “Tiger steel” in the
boxing industry. The status of his condition is critically unconscious at the ICU of Dr.
Elmon Cruz Las Pinas Hospital.

3. The suspect, Jaymark Conception, legal age, resident of Quezon City in Metro Manila
is currently detaining at Bacoor Police station and the case of murder under article 248
of the Revised Penal Code is pending for trial.

4. At about 9:00 pm of March 29, 2024 received a call from a certain person who
declare himself “Renz venuva” with CP no. 09638705066 requesting for police
assistance pertaining to an unconscious person at the Saint Josapin Hotel Talaba 1
Emilio Aguinaldo Hwy/ Longos Flyover Intersection Bacoor, Cavite room 30. Upon
verification of the initial facts this station immediately dispatched a team led by Police
Captain Delgado, Cristopher, Pcms Antonio tulidad, Psms jayson mandayag, Pmsg joey
sanbatuan and scene of the Crime Operatives (SOCO) led by PCINSP Joshua loboro.
Arriving at the Bacoor Saint Josapin Hotel, we were led to the scene of the incident by a
person identify himself Renz Venuva who owner of the said hotel and also introduced
himself as the caller and quickly led us to the room 30 where the victim was said to
have been found unconscious. We then asked Mr. Renz Venuva questions regarding
the incident and about the person who were in the room as we quickly restricted the
area in preparation of the investigation. Later, we approach the security personnel of the
hotel to let us view and later on secure for us a copy of the CCTV recordings for further
investigation and evidence.

Upon viewing the CCTV recording, at 8:40 pm from the clip, it could be seen that a lone
man with a black bracelet in his left hand was found entering the room of the victim with
gray small bag of the specifications consistent with what was found in the room 30 of
the victim. We immediately searched the area for such person but said person could no
longer be found at that time but because of our investigation we identified the suspect at
the grocery of Mang Kanor and also vendor of the black bracelet wear by the suspect
and fortunely mang kanor know where he is and quickly arrive to the suspect location
then arrested by our team.

5. The Bacoor Police Station coordinated with the SOCO team lead by PCINSP Joshua
loboro processed the crime scene and recovered the following items/evidences were
taken from the vicinity of the area:

a.) black bracelet

b.) gray small bag

c.) Copy of the CCTV recordings
