Parental Consent

I, ( ) in good faith consents my

son/daughter ( ) to attend the Diocesan
Youth Camp on July 17-21, 2023 to be held at New Mabuhay
Elementary School, Purok 4, Brgy. Mabuhay, General Santos City.

I have understand, that the YIFI Officers and the Parish

Priest shall not hold any accountability to any untoward incidents
that may happen, provided that they have conducted orientation
on the necessary precautionary measures of the event.

I warrant that, my son/daughter shall be provided with

modest accommodation and security upon its participation as part
of the requirements of the event.
