 Valid Passport validity 3 months after date of return ☐

(Two blank FACING pages and passport signed)
Temporary passport must not be older than 2 months from issue date

 Completed application form in duplicate ☐

 Two colour passport-size photos – biometric specification ☐

 Valid return air ticket and itinerary ☐

 Proof of medical insurance valid in Schengen countries ☐


 Original or certified copy of latest 3 months bank statements ☐

 Accommodation – Proof of hotel booking direct from hotel in Spain ☐

Business Travel

 Invitation letter from Spanish Company explaining nature of business ☐

 Letter from South African Company explaining nature of business, date

of travel and confirmation that costs are being covered ☐

 If self employed submit copy of company/CC registration papers and ☐

latest 3 months bank statements
Minors
Travelling alone or with only one parent – certified written consent from both
parents together with certified copy of ID/passports.
Unabridged birth certificate required.
