Liver function test (L.F.T)


*Liver :is organ have many function as :

1/Metabolic function : cholesterol ,Protein and CHO


2/Excretory function :Serum bilirubin, urine bilirubin

3/Synthetic function: Albumin. globin, coagulation


factor(Prothrombin) , Acute phase protein CRP
4/Detoxification function :determine of blood ammonia

5/ Storage function: vitamin k ,ferritin


6/ Hormonal secretion :thrombopoietin, angiotensin

Liver disease .
*Infection :Viruses and parasite
*Immune system abnormality :autoimmune .

*Genetics :hemochromatosis, Wilson's disease


*Cancers and other growth: Liver cancer ,bile duct cancer .

*TEST OF LIVER FUNCTION


1/Alanine transaminase (ALT)Or Glutamic pyruvic transaminase
(GPT)
2/Aspartate transaminase (AST)Or Glutamic oxaloacetate
transaminase (GOT).
3/Alkaline phosphatase (ALP)
4/Gamma –Glutamyle transferase (GGT)

5/Total bilirubin (direct +indirect )


6/Total protein and albumin

*Alanine transaminase (ALT)Or Glutamic pyruvic transaminase


(GPT)

:Is enzyme found in cystol of cell of many tissue ,but in


liver found in high concentration ,so is act more specific for liver.
Appears in plasma many days before clinical signs appear
it catalyze the transfer of an amino group from alanine to .
Alpha –ketoglutarate in the alanine cycle to form pyruvate and
glutamate .Half life :47 hour

3/Diagnosis of liver disease


4/Follow up or monitoring of patient who has hepatitis .

Male :up to 45 U/L

Female :up to 37U/L

Alanine +a.ketoglutarate---- ALT Pyruvate +Glutamate

Pyruvate +NADH+H___-LDH-Lactate +NAD+

1ml of reagent +100 micron of sample

:A3-A2=B , A2-A1=C , B+C/2=D

1. Sever elevation Up to 10 x times than normal ( 300-1000U/L)
