1. Enroll in ITE197 or ISY197
2. Join the SOJT Facebook GC
3. Secure Company Details
4. Fill-out Endorsement Letter
5. Secure Memorandum of Agreement (MOA)/Letter of Acceptance
6. Secure Waiver (If required by the company)

 Things to do (Post-OJT)
1. Submit Certificate of Completion (490 to 700 hours)
2. Submit Evaluation Form (filled-out by your Supervisor and sealed)
3. Clearance

 Reminders:
1. Apply for an IT/IS related company. If not, ask of your task
list/description of IT/IS project you are to do.
2. After being accepted, submit MOA for checking. Have it checked and
signed by the company personnel, you, your guardian, and submit four
copies to the department. We will have it notarized (prepare P50
payment).
3. At the end of your 490/700 hours, you will be evaluated by the
company (if they do not have an evaluation form, we have one) and do
not forget your OJT Certificate of Completion (this is important for your
graduation, submit hard copy to OJT Coordinator and soft copy to the
shared drive).
