1. In pursuance of Sub-Section (1) and Sub-Section (3) of section 26 of the Representation of the People
Act, 1963 (43 of 1951), I hereby appoint the officer specified in column 2 of the table below as Other
Polling Officer for the conduct of General Elections to HOP – 2024.

1. Failure to attend the election duty will be viewed seriously. Dissenting Officers are liable for disciplinary
action and are punishable as per Representation of the People Act, 1951.
2. Application Form (12)/(12A) for Postal Ballot is enclosed. The same may be duly filled and submitted at
the Facilitation Center. Please come with your EPIC (Voter ID) card Number, Voter enrollment AC, Part
No. and Sl. No. in Part as checked in https://voters.eci.gov.in/website.
