b. Issues & Concerns - Philhealth staff just checked and collected PMRF
Forms and are not able to verify attachments or
encode data on the system due to time constraints.
- Limited medications available

c. Recommendations/ Agreements - Philhealth instructed Barangay Captains to provide


list of individuals lacking attachments once they start
encoding data on the system
- Individuals with lacking attachments will provide
required documents to complete Philhealth
enrollment to Konsulta Package.
- Barangay will be awaiting for the list from the
Philhealth Office for immediate action.
- Coordinate with Philhealth for the Masterlist of
enrolled and lacking attachments for immediate
compliance.
- Include allocation of budget for purchasing essential
medications during the Workshop for the Creation of
Barangay Health Plan.

d. Analysis of the Activity There are so many individuals who wanted to update
or enroll to Philhealth but due to constraints in time
and finance, the attachments required may be lacking
and needs to be complied in a given time period.
