Application for Change of Surname

I SHAMPA SIKDER BISWAS OR SHAMPA BISWAS SIKDER, wife of TARUN KUMAR

BISWAS R/o AMLAPARA, BONGAON, NORTH 24 PARGANAS, WEST BENGAL, INDIA, PIN
- 743235, have changed my Surname from SIKDER BISWAS OR BISWAS SIKDER to SIKDER vide
Affidavit No. 96AB 034246 sworn before the LD.1ST CLASS METROPOLITAN MAGISTRATE AT
CALCUTTA.
Henceforth, I shall be known as SHAMPA SIKDER instead of SHAMPA SIKDER BISWAS OR
SHAMPA BISWAS SIKDER and all my relevant documents should be corrected accordingly. It is
further stated that SHAMPA SIKDER BISWAS OR SHAMPA BISWAS SIKDER and SHAMPA
SIKDER is the same and one identical person.

The information published in this Gazette Notification has been provided by me, this is true & correct to the best of my knowledge and belief.
If any legal issue arises in this regard at any stage, I shall be personally responsible for any consequences arising therefrom.
