Application for Change of Name

I ASOK KUMAR BHATTACHARYYA, son of PRATAP CHANDRA BHATTACHARYYA R/o

162B/318 LAKE GARDENS KOLKATA-700045 WESTBENGAL, have changed my Name from
A.K.BHATTACHARYYA to ASOK KUMAR BHATTACHARYYA vide Affidavit No. 64088 sworn
before the 1ST CLASS MAGISTRATE ALIPORE.
Henceforth, I shall be known as ASOK KUMAR BHATTACHARYYA instead of
A.K.BHATTACHARYYA and all my relevant documents should be corrected accordingly. It is further
stated that A.K.BHATTACHARYYA and ASOK KUMAR BHATTACHARYYA is the same and one
identical person.

The information published in this Gazette Notification has been provided by me, this is true & correct to the best of my knowledge and belief.
If any legal issue arises in this regard at any stage, I shall be personally responsible for any consequences arising therefrom.
