Application for Change of Name

I BASANTA DAS, son of GOUR CHANDRA DAS R/o SARSUNA SONAMUKHI ROAD
DASPARA VTC SARSUNA PO SARSUNA SOUTH 24 PARAGNAS PINCODE-700061, have
changed my Name from BASANTA DAS to BHOLANATH DAS vide Affidavit No. 20244 sworn
before the 1ST CLASS JUDICIAL MEGISTRATE ALIPORE.
Henceforth, I shall be known as BHOLANATH DAS instead of BASANTA DAS and all my relevant
documents should be corrected accordingly. It is further stated that BHOLANATH DAS and
BASANTA DAS is the same and one identical person.

The information published in this Gazette Notification has been provided by me, this is true & correct to the best of my knowledge and belief.
If any legal issue arises in this regard at any stage, I shall be personally responsible for any consequences arising therefrom.
