Dear Branch Chief Orlikowski

This letter is a request by Midwest Bio-Products to replace our current RSO, Masakazu Miyagi, PhD
with Heather Y. Miyagi, DVM, PhD. Over the past 10 years or so, she has been assuming more and
fTlOre of the work that he had been doing since 1981 , and she is fully qualified to fill the position.
Masakazu Miyagi wishes to retire from the official RSO position, but will remain available for
consultation as needed.

If we do not hear to the contrary, we will list Dr. H.Y. Miyagi as our RSO in our next license renewal
which is due in 2024. Thank you for your consideration.
