Research Title: The Impact of Mindfulness Meditation on Cognitive Performance: A

Randomized Controlled Trial Study

Introduction:
You are invited to participate in a research study to investigate the impact of mindfulness
meditation on cognitive performance of senior high school students. Before you decide to
participate, it is important that you understand the nature of the study, its purpose, and the
procedures involved.

Purpose:
The purpose of this study is to examine how mindfulness meditation can impact the cognitive
performance of senior high school students. This research aims to contribute valuable insights
into effective strategies on improving cognitive performance using mindfulness meditation.

Procedures:
If you agree to participate, you will be asked to:
- Attend the daily mindfulness meditation sessions. A 10 day session starting on April 8 to April
19. Please note that these sessions will happen during class hours therefore you will be missing
at least 10-20 minutes of a class lesson.
- Follow the researcher’s instructions during mindfulness meditation.
- Provide information through survey questionnaires.

Confidentiality:
All information collected during the study will be kept confidential. Your identity will be
protected, and data will be anonymized to ensure privacy. Only the research team will have
access to the data.

Voluntary Participation:
Participation in this study is entirely voluntary. You may choose not to participate or withdraw
from the study at any time without penalty. Your decision will not affect your current or future
relationship with your employer.

Consent:
I have read and understood the information provided in this consent form. I have had the
opportunity to ask questions, and any queries have been addressed to my satisfaction. By
agreeing to participate, I willingly consent to be part of this research study.
