Incidence: While working near SSP, Insulator Pipe cutting, Cutting bledge broken and hitted
gangleader forehead. Resulting in injury on forehead.
Findings: Incompetent workmen deployed for cutting activity.

Immediate cause:

1. Cutting blade found in bad condition.

2. Also, no Pre-checking done before using at site.
3. No competent workmen deployed for cutting work.

Basic cause: Incompetent workmen deployed for cutting work at site, also blade condition found
bad.

Workers’ participation and consultation:

1) MR ANUJ YADAV
2) MR MITHLESH PATEL

Recommendation/Corrective Action:

Before deploying workmen at site, Site Engineer should provide proper training or tool box talk
regarding particular activity.

Should deploy competent cutting person for cutting activity.

All workmen should use proper PPE’s while working at site.

Proper intimation and coordination regarding work should be provided.
