have to use it starting October 1, 2019, except for the Risk Vulnerability Checklist.

will go out of your respective Stations to Conduct OLP Activities.
 ALWAYS ask the Barangay Chairman or any Representative to sign the
Acknowledgement portion.

3. CLIENT SATISFACTION SURVEY FORM

 1 FORM PER HOUSEHOLD
 The TITLE SHOULD BE CLIENT SATISFACTION SURVEY FORM
 Please download the updated FORM that I sent together with this post.

4. RISK VULNERABILITY CHECKLIST

 Used during first Quarter in the conduct of CFPP.
