1. Request conduct quantitative and qualitative examination on the specimen

submitted for the determination of presence of dangerous drug.

2. Facts of the Case:

a. Nature of Case: Violation of Section 5 in Relation to Section 26(b) and

Section 11, Article ll of RA 9165 Otherwise Known as “The
Comprehensive Dangerous Drugs Act of 2002” as amended by R.A
10640.

b. TDPO: Around 6:15 PM of April 25, 2022 at Molino Blvd, Mambog IV,
Bacoor, Cavite.

c. Arresting Officers: PCpl Albert Ramos (Poseur Buyer/Seizing Officer).

d. Investigator-On-Case: PCpl Renz T Suarez and Pat Marvin G Veniegas

3. Name of Suspects:

a. Isaac Gabriel Ambulo y Paidon AKA Taba, 29 years old, single, jobless,
and a resident of Quiapo, Manila;

b. Roman Hosias Ambulo y Paydon, 25 years old, single, tricycle driver,

and a resident of Quiapo, Manila; and

c. Abdurrahim Ambulo y Disomimba, 41 years old, married, jobless, and a

resident of San Fernando, Pampanga.

4. Evidence Submitted:

One (1) open color white plastic bag marked as ADR 6:15 PM 04/25/2022
MOLINO BLVD. BACOOR CAVITE with signature containing one (1) opened
transparent plastic pack marked as ADR-1 6:15PM 04/25/22 MOLINO BLVD BACOOR
CAVITE with signature further containing one (1) opened black/gold foil pack with label
Coffee Espresso marked as ADR-2 6:15PM 04/25/22 MOLINO BLVD BACOOR
CAVITE with signature on masking tape further containing one (1) heat-sealed
transparent plastic pack marked as ADR-3 6:15PM MOLINO BACOOR CAVITE
04/25/22 with signature further containing one (1) vacuum heat-sealed transparent
plastic pack further containing white crystalline substance believed to be shabu.
