for his/her welfare and safety. I fully agree to waive any responsibility on the part of
Cavite State University-Bacoor Campus, Military Science and Tactics, the concerned
partner agency/institution and/ or of its representatives, in any case of any untoward
incident that may happen to my daughter/son during the conduct of the training.
