Subject: Waiver for Participation in Robotics Training Program

Objectives: The program aims to provide hands-on experience in robotics while fostering essential skills in
STEM fields.

To whom it may concern,

I hereby acknowledge that my child, ____________________________, will be participating in the robotics
training program, organized by the Science Department, scheduled to take place from April 11-12, 2024 at
Cabiao Senior High School Conference Hall. The Registration fee amounting to 250.00 each will be used for
the preparation of the program and for the guest speakers.
I certify that my child is in good health and capable of participating in the program activities. I agree to
abide by all rules, regulations, and instructions provided by the program organizers.
By signing below, I acknowledge that I have read and understood the contents of this waiver, and I
voluntarily consent to my child’s participation in the robotics training program.
