PROBATIONARY CONTRACT

KNOW ALL BY MEN THESE PRESENTS:

This AGREEMENT made and executed by and between LA ROSA BEAUTY
LOUNGE with business address at, Door 7 Grand Emerald Plaza, Zone 6, San Felipe, Naga
City, herein represented by its owner MR. JOHN RHONLIE O. ORTEGA, of legal age,
married, and with office address at No. 81 Zone 6, San Felipe, Naga City hereinafter referred
to us the First Party;
-and-
___________________________, of legal age Filipino, single/ married, and a
resident at ___________________________ hereinafter referred to us the Second Party;
WTNESSETH
1. That the daily rate of the Second Party is THREE HUNDRED SIXTY-FIVE (365)
PESOS;

2. That the Second Party is entitled for 10% commission for nails and facial and 20% for
massage services;

3. That she is designated as __________________;

4. That the Second Party agrees with the following conditions once employed:

a. She maintains no direct contact and home service with the customers;
b. She is not allowed to share the process of the business even if she is no
longer connected with the company;
c. She is also liable for the damages and loss of the equipment used in the
SPA business;
d. She is obliged to wear her complete uniform during work hours;
e. She must be courteous and respective to all customers at all times;
f. Maintains P3,000.00 cash bond being mandatory for her continued
employment;
g. She also agrees to notify the first Party within 30 days prior to her
resignation otherwise her cash bond will be forfeited in favor to the
First Party and the law provides that such failure to notify their
resignation, the employer may file an action for damages before
appropriate tribunal;
h. She obliges to surrender the uniform issued by the First Party on the
last day of her work;
i. Service procedure should be correct and step-by-step / if not - subject
for memo;
j. Disobeying the management will be subjected to a memo, with 3
grounds, ends to termination;

5. That this agreement supersedes any other agreement among the PARTIES whether
written or oral previously entered into by the PARTIES and any conflict that may
arise between the provision of this agreement and previous one, the provisions of this
agreement shall prevail.
 IN WITNESS WHEREOF, we have hereunto affixed our signature this ____ day of
