Ms. W.R is 18 years old girl and the only child of the family. She had completed her A-level from
Dubai. She belongs to upper class nuclear family system and used to live with her parents in Dubai
and she had shifted to Pakistan last year.
Mode of Referral
Client was brought by her mother in hospital because her daughter faces fear and increase in heart
rate near close places and elevator.
Presenting Complaints
Clients said that “from past 8 to 10 months
 I develop a fear of using elevator

 My heart rate start increasing

 Use to take long breathes and start sweeting

 I feared a lot now a days and use to avoid taking elevator

 Past four months I am feeling same things in other close places.

History of Present Illness

Client report that 9 month ago she stuck in an elevator in a mall for half an hour after this event these
things are happening to her. She feels fear and thinks that if it’s happened again what I will do.
Before this incident everything was good and she use elevator and everything was normal.

Developmental History
She was born with the normal delivery and had completed all stages very well. Her mother tells us
that she was quick learner and obedient girl throughout her life.
Educational History
She was good student throughout her life she was brilliant in studies and learning and delivering new
thing. Her behavior was good with teachers and classmates she also play badminton with her
classmates.
Medical and Psychiatric History
She had no medical and psychiatric history.
Family History
Client is the only child of the family. Her father Mr. A R is 50 year old and run real estate business in
Dubai and Pakistan. He had done bachelors in economics. Her mother Ms. F R is 46 year old house
wife. She also had bachelors in economics. They both were class fellows and after their graduation
they take decision to get married. They had no major medical and psychological issues. Home
atmosphere is quiet good and supportive and fun loving, they all are having good life.
