JOB ORDER CONTRACT
KNOW ALL MEN BY THESE PRESENTS:
This job Contract is entered into this 9 th day of January 2024, DepEd Schools
Division of Apayao, Luna, Apayao, by and between:
The Schools Division Office of Apayao, a government entity, with office address at
Capagaypayan, Luna, Apayao, duly represented by Schools Division Superintendent Irene S.
Angway PhD, CESO V, herein referred to as the “First Party”.
-and-
Jethro Balyao, single/married, of legal age, Filipino, and a resident of Baliwanan, Kabugao,
Apayao, herein referred to as the “Second Party”.
WITNESSETH
WHEREAS, the Schools Division Superintendent, Assistant Schools Division
Superintendent, and the Administrative Officer V will conduct a monitoring and school visit
at Kumao ES, Dangla ES, and Baliwanan ES on January 10, 2024;
WHEREAS, the purpose of the visit is to meet with the teachers and barangay
officials of Kumao and Baliwanan to discuss the possibility of establishing an integrated
school;
WHEREAS, along with the monitoring and school visit will be the distribution of
books donated by benefactors from Book Wermz;
WHEREAS, these books are intended to serve as supplementary reading materials
and references/learning resources for students;
WHEREAS, this initiative aims to enhance the overall educational experience by
providing students with diverse and enriching content, fostering a love for reading;
WHEREAS, the only means of transportation to reach Kumao ES, Dangla ES, and
Baliwanan ES is by riding a single motor;
WHEREAS, a single motor is needed to transport the books/reading materials to the
intended schools in Kumao ES, Dangla ES, and Baliwanan ES.
NOW THEREFORE, the parties hereby agree as follows:
TERMS AND CONDITIONS
A. That I agree to enter into a Job Order Contract arrangement with the Schools Division
Office of Apayao effective January 10, 2024 in consideration for Two Thousand Pesos
(Php2,000.00), to be paid after the travel and upon submission of Certificate of Appearance,
Accomplishment Report, and other pertinent documents.
B. That I commit to perform the following function/s:
1. Pick up books and reading materials from the Poblacion, then transport them to
Kuma ES, Dangla ES, and Baliwanan ES.
C. That the service under this Job Order Contract, shall not be accredited as government
service and shall not create an employer-employee relationship.
D. That this contract shall remain valid within the time frame specified in Item “A” but
can be terminated by the Schools Division Office of Apayao at its own discretion motu
propio.
IN WITNESS WHEREOF, the parties hereunto signed this Job Order Contract, this
9th day of January 2024 at DepEd Schools Division of Apayao, Luna, Apayao.
