Investigation for Accident No (1) :


Question : Describe in as much detail as possible, what happened exactly ?
Answer : While I was standing in front of “ICAD warehouse / backside of ICAD site office” after arranging the scrap area, in this
time the AES consultant’s safety officer -Mr. Ali- comes to me and push me in left back shoulder and shouted me and asked me to go
back to work in the same area.
Question : When exactly did the accident happen?
Answer : In Wednesday, 12th of June 2024, morning , properly around 09:00 AM

Answer : While I was working in Admin Building first floor in floor chipping and I was wearing ear plugs, in this time the AES
consultant’s safety officer -Mr. Ali- come to me and he said something and I didn’t hear because of I was wearing the ear plugs, in
this time he kicked me twice by his foot to bring my attention to him to stop the work.

Answer : In Thursday, 13th of June 2024, properly around 11:00 to 11:30 AM
