Key Roles In WMS.

Asset Owner , Typical Example, Senior Vise President Operations or vise president operations .

Description, Person responsible and accountable for the integrity and safe conditions of the complete
structure and facility, system and equipment within allocated to him.

Asset Approval authority (AAA) , Typical example, Operation manager , duty manager.

Description, A competent person approved in writing by asset owner who shall sign all critical and hot
work permits, confirming that the described controls are adequate to reduce the risks to As Low As
Reasonably Practicable (ALARP).

Area authority (AA), Typical example, Shift Superintendence, Shift Supervisor, Shift Controller .

Description, A competent person approved in writing by asset owner and is responsible for monitoring
the safety of an area and the asset within it and providing clearance to work in that area at any
particular time.

Permit Issuer (PI), Typical example, Field Operator, Senior Field Operator, Outside Area Operator.

Description, A competent person approved in writing by asset owner and is responsible for monitoring
the safety of an area and the asset within it and providing clearance to work in that area at any
particular time.

Performing Authority (PA) , Typical example, Maintenance Supervisor, Wells Supervisor.

Description, A competent person within the PTW system, assign to request a permit or supervise job
performers. This person understands the task and accepts responsibility for ensuring that details in the
permit and associated attachment provide a clear description of the work to be performed including
tools, materials and any specific skills required.

Job Performer (JP) , Typical example, Maintenance Technician .

Description, A person who works under direction of the Performing Authority and accepts the worksite
as safe to commence the work, he communicates the Hazards, permit control and conditions to other
work party members or group or personnel.

Permit Endorsers (PE) , Typical example, Fire chief, Specialist, interface area authority, interface area
operator.

Description, Permit Endorser are personnel who are affected/ impacted by activities described in work
permit (Other than IA, JP,AAA and PI of the same Permit ) or personnel whose expertise may be
necessary for defining, approving controls. Permit endorser review the work scope and the control
identified to mitigate interface , concurrent activities and agree that the work described may be carried
out during the proposed period.
The use of permit to work crash pack shall be authorize by Asset owner with endorsement from site HSE
manager.

In permit to work daily meeting area authority (AA) shall review the current number of Permits with
respect to the permit to work limits, prior to accepting new permits.

For all critical and hot work permits , Area authority (AA) shall validate that all details in the permits and
associated documents are clear and valid to proceed further for endorsement and asset approval
authority (AAA) approval.

Hot and critical permits required asset approval authority (AAA) , however the AAA approval may be
exempted for hot work permits in non-process or unrestricted area.

Performing authority (PA) worksite presence determined by Area authority (AA).

Permit endorses (PE) sign the permit as applicable.

All pre-requisite worksite controls shall be implemented as per permits requirements and subsequently
confirmed by Area authority (AA) as I place and effective.

Verification of the effective implementation of controls shall be provided by permit issuer (PI) and
isolating authority (ISA).

The work shall only commence post on-site clearance by the permit issuer (PI).

For general work permit, Area authority (AA) shall authorize the permit in advance.

Authorize gas tester (AGT) with valid certificate shall conduct the atmospheric test prior to
commencement of work.

The permit issuer (PI) shall demonstrate zero energy to job performer (JP) confirming worksite is safe to
commence the work.

The original copy of the permit shall be kept in the facility control room, wrong.

The original copy of PTW issued to job performer JP and displayed at worksite.

Fist copy retained by area authority AA at point of issues.

Second copy retained in control room if different from point of issues.

The periodic review of Job Safety Analysis is 2 years.

Who is responsible to develop Formal risk assessment, Team Leader.

The requirement to conduct independent verification of each isolation point is determined by , Isolating
Authority ( ISA),or (AA).

Positive isolation type shall be used for confined space, False.

Who is responsible to develop the energy isolation plan / Scheme, Isolation Authority (ISA).

Sanction To test ( STT) is required to de isolate the equipment for test purpose and shall be approved by
Area Authority, True.
The activity required for mechanical isolation, Confined space entry and closing valve, removing spool,
blinding.

The two types if defeat as per WMS standards are, Process defeat and Maintenance defeat.

The appropriate defeat approval authority as per the followings.

Individual device / unit for 12 hours to 72hours.Team leader, section lead, operation.

SIL 3 devices or systems for more than 72 hours. SVP, VP operations.

SIL 2 devices or systems for more than 12 hours to 72 hours. Manager operations.

Individual device / unit for less than 12 hours. Area Authority.

Who is the certificate applicant for the temporary defeat certificate for process defeat. DCS supervisor.

Who is the certificate applicant for the temporary defeat certificate for Maintenance defeat.
Maintenance supervisor.

Two condition that are related to or linked to Simultaneous operations (SIMOPS). Activities done at the
same time, Activities done at the same area.

The Two type of Simultaneous operations (SIMOPS) as per WMS system. Routine and Non-Routine
SIMPOS.

The three responsibilities of the Simultaneous Operations Coordinators are. Prepare Simultaneous
Operations Plan, Communicating with Authority, ensure all safety precaution and controls are
implemented.

Tier1,daily individual permit or other WMS documentation shall be monitored, (Self audit).

Tier2, annual audit program to assess the effectiveness of complete WMS standard by typically
corporate HSE personnel or leader ship team within the group companies.

Tier3, independent as specified by adnoc audit guideline.
