AFFIDAVIT OF LOSS

I, Pan, Zhiwen, of legal age, Chinese citizen, residing at Unit 205 Tower C Shell
Residences Pasay City, under oath, depose and state:

(1) That I am a holder of a People’s Republic of China Passport with no. E59115544
issued by the MPS Exit & Entry Administration.

(2) That on August 2023, the said Passport was lost, apparently it was misplaced or
dropped unnoticed somewhere in the vicinity of Intramuros, Manila.

(3) That despite diligent efforts to recover the same, all were in vain;

(4) That I am executing this affidavit to attest the veracity of the foregoing facts and
for other lawful purposes to comply with the requirements of Bureau of Immigration for my
application of Annual report and ICARD Certification.

Affiant further sayeth naught.
