AFFIDAVIT OF CHANGE OF NAME


I, MRS. OTEH MABEL, a native of Rumuakunde Emohua in Emohua Local
Government Area of Rivers State, an Adult Female, Citizen of the Federal
Republic of Nigeria, do hereby make an oath solemly declare as follows:

1. That I am deponent herein and swear to this affidavit in good faith.


2. That I was formerly known, called and addressed as MRS. OTEH MABEL.
3. That I now want to be known, called and addressed as MISS. OKERE
MABEL EGO.
4. That this change of name is as a result of my NIN bearing the name MISS.
OKERE MABEL EGO and my BVN bearing MRS. OTEH MABEL,
hence this affidavit.
5. That I swear to this affidavit for record purposes and for the information of
all concerned authorities to take note

AND THAT I, make this solemn declaration conscientiously believing same to


be true and correct by virtue of the provisions of oath acts 2004.
