AFFIDAVIT OF CORRECTION OF DATE OF BIRTH

I, MOSES CHIMENEM ROMANUS, adult, Male, Student, Christian, citizen of the

Federal Republic of Nigeria, of No. 10 Igwuruta Road, Rumuowegwu Eneka , Port
Harcourt , Obio/Akpor Local Government Area in Rivers State, do hereby make
oath and declare as follows:
1. That I am the deponent herein and depose to this affidavit in good faith.
2. That I am operating an account with Access Bank Plc with Account No.
1673305621.
3. That also I am the bonafide owner of the NATIONAL IDENTIFICATION NUMBER SLIP
(NINS) with NIN: 90682955103 which was issued to me by the National Identity
Management System, Federal Republic of Nigeria
4. That on the said Bank Account and NIN details, my Date of Birth was
erroneously written as 8th day of August, 2003 instead of 8th day of August,
2007.
5. That my true and correct Date of Birth is 8th day of August, 2007.
6. That this affidavit is made for record purposes and for the information of
whosoever it may concern.

That I make this solemn declaration conscientiously believing same to be true and
correct by the provisions of the Oaths ACT, 2004.
