C. Presenting Problem

The client is a 34-year-old female that is dealing with a goiter disease and seeking
medical assistance.
D. Background Information

Client and Family History

Aying is from Purok 25, Poblacion, Magsaysay, Davao del Sur, and she is
the fifth child in her family. She graduated from Monkayo National High School. In 2012,
while working as a helper at her cousin’s house in Tagum, she met Takoy, who would later
become her live-in partner. That same year, they began living together and were eventually
blessed with three children: Sin John, Andree, and Inday. With the guidance and support of
their parents, the children are being raised well and are currently attending school—Sin
John is in Grade 6, Andree is in Grade 1, and Inday is in kindergarten.

Aying is originally from Monkayo, Davao de Oro. In 2012, at the age of 21, she
moved to Tagum City to work as a helper at her cousin's house. While living there, she met
her live-in partner, Takoy, through their neighbor—who happened to be related to him. On
December 3, 2012, Typhoon Pablo struck, which led the couple to relocate to Davao City.
In 2013, they decided to move again—this time to Purok 25, Poblacion, Magsaysay, Davao
del Sur—where Takoy originally came from. Aying and her partner lived a normal and
stable life together for several years. Like most couples, they experienced occasional
arguments, but these were usually resolved quickly. Their first child, a son named Sin
John, was born on July 15, 2014. They later welcomed two more children: Andree in 2019
and Inday in 2020. The house they are currently living in was given to them by Takoy’s
brothers. Since the brothers were already married and had their own homes, they decided
to give up their rights to the family house and transferred ownership to Takoy and Aying.
This arrangement was made official through a signed contract.

Environment and Community


The clients currently reside in their own home located in Purok 25, Poblacion,
Magsaysay, Davao del Sur. The house is made of full concrete and features plain glass
windows. Their water supply comes from the municipal line, accessed through a hose and
faucet, which they use for drinking, bathing, and doing laundry. Aying enjoys gardening,
especially growing vegetables, as a way to save money and support their daily needs.
Although they live in a somewhat crowded neighborhood, the community is known to be
friendly and hospitable. The family's main mode of transportation is the tricycle. Within
the community, some houses are built with concrete materials, while others are made of
lighter materials. Despite the differences in housing, the area remains simple and
peaceful, with residents living harmoniously.
Social and Economic Condition
Aying, a 34-year-old woman, is currently struggling to manage her responsibilities
as a housewife while also dealing with a goiter condition. Her live-in partner is the sole
breadwinner of the family and earns a weekly income of ₱4,000 to ₱5,000, depending on
available work. This amount is just enough to cover the family's daily food needs and is
not sufficient to support Aying's medical treatment for her condition. Fortunately, the
family is a beneficiary of the government's 4Ps (Pantawid Pamilyang Pilipino Program),
which provides financial aid. However, they only receive ₱3,300 every two months,
which must be allocated for their children’s education and other basic needs. In addition
to these expenses, they also have to pay for utilities such as water and electricity. Due to
these financial limitations, Aying has not been able to seek any proper medical attention
for her goiter.

E. Student's Assessment
The client live-in partner income is only enough for there daily needs food, water,
clothing, and electricity. Due to this she can't check afford to check the condition of her
goiter. In further observations she is having also a hard time dealing with her daily house
chores along with being a mother of three it really builds up stress that she claims that is
making her goiter worse. Although she can still move [Link], it is observed that
she lacks interest of making her self better and relieved from her goiter. Due to this the
client is eligible to receive medical assistance because she her live-in-partner along with
the 4ps can't provide her medical needs.

This case is supported by Systems Theory, which emphasizes how different


aspects of a person’s life—such as family, community, and social support—are
interconnected and influence one another. In Aying’s case, her health condition is not
just a personal issue but is affected by her family’s financial limitations, her caregiving
responsibilities, and limited access to healthcare. These systems interact and contribute
to her hesitation in seeking medical treatment.

Based on the student field worker’s observations, her decision-making is shaped


by the pressures within these systems. It is the role of the social worker to help Aying
understand the importance of seeking medical care, to strengthen her support system,
and to connect her with resources that can assist in addressing both her health and
overall well-being.

II. PLAN OF ACTION

III. INTERVENTION OR PLAN IMPLEMENTATION

May 04, 2025 student field worker conducted a home visitation to gain deeper
understand of the client’s condition. On May 10, 2025, the student field worker
accompanied Aying to inquire at the Municipal Health Center of Magsaysay to assist the
client in obtaining medical assistance from an agency . May 15, 2025 the student field
worker assisted the client in compiling and processing all necessary documents for a
formal medical assistance request. This included gathering medical certificates,
identification documents, and application forms. The student accompanied the client to the
appropriate offices, ensuring that all required documents were submitted on time. On July
15, 2025 the student field worker assist the client on the day of her surgery for her goiter.
This included follow-up visits, emotional support, transportation assistance for check-ups,
and ensuring access to prescribed medications. The client experienced a smooth and
supported recovery. On July 27, 2025,the student field worker accompanied the client to
the DSWD office to inquire about and apply for financial assistance through the AICS
program. The student guided the client through the steps of application and documentation.
As a result, the client was granted financial support to help cover her maintenance
medications and other related expenses.
IV. EVALUATION AND RECOMMENDATION

The client, Aying, is a live-in partner to Takoy and the mother of three children.
Takoy, who owns and works as a butcher at a meat shop, earns just enough to provide their
family with three meals a day. This leaves Aying unable to seek medical care for her
condition. Fortunately, their family is a beneficiary of the 4Ps program, which provides
some subsidies, though these funds are insufficient to meet Aying's medical needs. The
student field worker observed that Aying struggles with balancing her responsibilities as a
mother and managing household chores, which has placed significant stress on her body,
exacerbating her goiter condition. Despite these challenges, Aying remains hopeful that her
goiter can be treated with proper medical care.

A significant milestone occurred during a free check-up at the Municipal Health


Center, where Aying's goiter was examined. Additionally, the client received free
medication from the RHU of Magsaysay, which alleviated her financial burden of
purchasing medicines. After two months of assistance, the student field workers noticed an
improvement in Aying’s overall health, especially her mental well-being. She appeared
more lively and was able to manage her housework better, knowing that her condition was
under treatment. This positive change was confirmed through the field workers'
observations.

Based on these findings from thorough interviews and home visits, the student field
workers concluded that Aying is eligible for continued medical assistance, which has
contributed to her improved health. She can now better manage her housework without the
constant worry of her untreated illness. The student field workers recommend that Aying
continue taking her prescribed medication and follow the instructions provided by the
RHU Magsaysay staff. They also suggest that the LGU Magsaysay consider offering
additional professional support to further assist Aying. Lastly, the student field workers
recommend that Aying seek a follow-up consultation with a doctor to gain a more detailed
understanding of her goiter’s condition.

V. TERMINATION
The case has been closed because the goals and objectives have been achieved. The
client has received medical help and is living her best life.
