MEDICAL REPORT
RE: IMAORUEN IKATA /MALE/ 28 YEARS

This is to inform you that the above named person suffers from Asthma and
Pneumonia. The disorder was diagnosed due to the patient’s complaint of chest pain,
stomach pain, cold, shortness of breath, fever, and vomiting. He has been placed on
rigorous regime of drugs and medical testing.

Due to this illness situation of extreme fever, immense cough and cold as well as the
environment, rigorous activities and excitement are to be avoided. Also medical
monitoring by physician is essential for the management of the illness. Hence it’s
therefore medically recommended that any situation that will place the patient under
strain should be avoided.

In view of the above, the bearer is hereby advised to be close to the doctor in case of
emergency and for proper medical care.
