Dear Madam,

We would like to request your good office to approve our Purchase request submitted
on November 26, 2024, for the 2-day District In-service Training for Teachers and 3-
day Division Training for Teachers. The delay of submission of PR is due to the conduct
of Scouting Activity, Celebration of Book Month and Sports Fest that we’ve had last
week. Furthermore, our AO also informed us last Friday afternoon that the PR we
should be submitting will be good for 2 months. Hence, we have changed the PR that we
previously prepared.

I hope that you will consider our request. Rest assured that this delay incident won’t
happen again. Thank you and God bless your leadership.

Very truly yours,

The student-led school watching and hazard mapping are initial steps
in engaging learners in disaster preparedness and management. This activity
will raise awareness and equip learners with information and skills necessary
for addressing the impact of hazards. Through this activity, the Department
of Education (DepEd) creates an opportunity for learners to understand their
role in disaster risk reduction and management (DRRM).
In Jose Rotea Memorial School, a group of students participated in
hazard mapping led by a teacher trained in DRRM, Mr. Javier M. Girasol, and
supported by their school Principal Mrs. Yvonne Irene S. Espinas. With the
strong sense of purpose, the students guided by Mr. Girasol, began their
assessment of the campus. They explored every corner, noting down
potential hazard such as:
 Broken window
 Slippery pathway
 Busted plugs/light bulbs/electrical facilities
 Exposed electrical wires
 Protruding nails in chairs and tables
 Dripping ceiling
 Stagnant water
 Unpruned trees/bushes/shrubs
 Open/clogged canals
 Broken/dilapidated ceiling
 Open/incomplete perimeter fence
 Presence of electrical post/transformer near or within the school
perimeter
 Garbage area (segregation of biodegradable and non-biodegradable)
 Detached or peeled off GI sheet
 Broken toilet bowl and/or sinks

 Broken chairs/desks/tables
 Unmounted cabinets/shelves
 Medical kits in every classrooms
 Condemnable building (i.e. very old structure, collapsing building
and/or prominent cracks on classroom walls)

In the end, the student-led school watching and hazard mapping not
only improved the students’ level of awareness and the school’s safety but
also taught the learners the value of teamwork and responsibility. Their
effort can make Jose Rotea Memorial School a safer place for everyone
through identifying potential hazard that could lead to its repair in the
future.

Dear Teachers and Learners,

Greetings of peace and love!

Just recently, the father of our learners namely Rica Mae Legarto
(Grade 3), Rheajane Marrie Legarto (Grade 5), and Reybe Legarto (Grade 5)
has met a vehicular accident. As one family, we are knocking at your kind
hearts to share any voluntary assistance to aid for the expenses needed by
their father. Your help, no matter how small can save a life.

Remember, no one has ever become poor by giving. Thank you for
your kindness and may God always keep us and our families safe.

Sincerely yours,

MASS DEWORMING
SY 2024-2025

School-age children typically have the highest intensity of worm infection of any
age group. Regular deworming contributes to good health and nutrition for children of
school age, which in turn leads to increased enrolment and attendance, reduced class
repetition, and increased educational attainment. The most disadvantaged children – such
as girls and the poor – often suffer most from ill health and malnutrition, and gain the
most benefit from deworming.
Before Deworming
The school prepared the enrolment list disseminating all parents’ consent to each
learner. The School Head has received the Albendozole tablets from the division nurse,
Mr. Daryll Torregosa.

During Deworming
On August 27, 2024, after all parents’ consents have been retrieved from the
learners, no parent’s consent, no tablet is strictly followed by the school. With the
assistance of Division Nurse, Mr. Torregosa, the chewable tablets were given personally to
learners. With the help of Mr. Torregosa, learners were dewormed successfully.

Some of the learners were dewormed in the barangay and some were not allowed by
their parents so there are still a few who were not dewormed.

Post Deworming
The Administrative Aide has recorded the Mass Deworming Activity to be submitted to
the Division Health Section for reporting.

Dear Sir/Madam:

Season’s greetings!

Christmas Party is a yearly celebration in Jose Rotea Memorial School (JRMS)

where pupils get to enjoy the company of one another before the holiday break. It is a
time of sharing and giving joy especially to children.

JRMS has decided to conduct a Christmas Carolling activity in order to generate an

amount to be used in buying food and gifts for our learners. We want to fill their hearts
with joy and laughter and make them feel how much they are loved.

In order for this goal to be achieved, we are sending you this letter to inform you
that our teaching and non-teaching staff will conduct a Christmas Caroling at your house
within December 9 - 13, 2024 between 4:00PM-6:00PM. Any amount you give will surely
give joy to our learners.

Advance Merry Christmas! Thank you and may our good Lord bless your generous
hearts.

Sir:

Greetings of peace!

Now that Valentine’s Day has concluded and February is about to end, we would
like to inquire about the possibility of repurposing any unused or reusable decorations
from the City’s celebration of Love month. Rather than letting them go to waste, we
would love the opportunity to use them for our school decoration.

Our school, Jose Rotea Memorial School, would love to recycle and repurpose
them to decorate our campus and create a more festive and welcoming environment for
our students.

We are hoping for your approval to this request. Thank you for your time and
support in promoting sustainability.

Sir:
Greetings!

Jose Rotea Memorial School has recently joined “Sustainable and eco-Friendly
Schools” contest. As part of our efforts to promote environmental sustainability, we, the
GPTA of Jose Rotea Memorial School are working on creating an eco-friendly pathway
on our campus.

We are kindly requesting your support in providing 30 footsteps for this

pathway which will help guide students and visitors through a sustainable route. The
steps will not only enhance the safety of our school but also serve as a visual
representation of our commitment to the environment.

We look forward to your positive response to this request.

Thank you and God Bless!

Dear Madam:

Greetings of peace and prosperity!

Jose Rotea Memorial School is truly grateful to be chosen as the recipient of the
Silliman University Library Reach Out (SU LibRO) project. This opportunity will
undoubtedly bring positive impact in the education of our learners.

Currently, the schedule is set from 8:30 AM to 10:00 AM on Friday, February 21, 2025,
but we believe it would be more beneficial if it will be moved to 2:00 PM to 3:30 PM on
the same day. Due to the intense heat in the morning, learners may find it difficult to
focus, and the intense sunlight may affect their overall participation. A later schedule
would provide a more comfortable environment, ensuring better engagement among
learners.

We hope for your kind consideration to this request. Thank you and more power!

Sincerely yours,

Dear Mayor/Sir,

Greetings from Lamwas Elementary School!

As part of our ongoing commitment to enhance arts education and school-wide events,
we are planning to construct a durable, properly equipped stage on our campus.
Currently, we rely on temporary or outdoor platforms that lack proper lighting, sound,
and safety features—making performances, ceremonies, and community events
challenging and occasionally risky.

Your contribution will go a long way in creating a better place of learning for our
students and in promoting the spirit of bayanihan in our community. We would be glad
to acknowledge your support during the program.

Should you have any questions or need further information, feel free to contact us
through this no. 09666549065

Thank you for considering our appeal. With your valuable support, we can build a safe
and inspiring center stage for current and future students.

Dear Ma’am

I am writing to respectfully request that I not be transferred from Jose Memorial School. I
have only been at this school for two years, and I believe that more time is needed for me to
make a lasting impact.

During my time here, I have begun several initiatives aimed at improving the school’s
performance and student welfare. These plans are still ongoing, and I am committed to seeing
them through for the benefit of the entire school community.

In addition, my two daughters are currently enrolled at Jose Memorial School. Being at the
same school allows me to support them closely in both their academic and personal
development.

I am very passionate about continuing my service at Jose Memorial School and contributing
positively to its growth. I kindly ask for your understanding and support in allowing me to
remain in my current position.

Thank you for your time and consideration.
