Consent to Collection and Use of Personal Data

I, ______________________________________________, an employee of Tradco Service Limited


(Employer/Company) unequivocally consents to having my personal data including but not limited to my
personal identification documents and images collected, handled, transferred and/or stored, as the case
may be, in accordance with the Employer’s policies on information security and data governance, and the
Data Protection Act, No. 24 of 2019 of the Laws of Kenya (Data Protection Act). I understand that the said
policies are accessible on the Employer’s intranet folders, and are available on request from the Human
Resource Manager.

I hereby consent to the use of my personal data including but not limited to my images/photographs taken
in the course of my employment with the Employer on the Company’s website, social media pages and/or
any other media.

I also confirm that I am aware that the Company is contracted by other third parties subsequent to which I
am employed by the Employer. Accordingly, I extend my consent and authorization to the use by such third
party on its website, social media pages and/or any other media.

I hereby waive any right(s) to make any claim(s) or to institute any legal proceedings before any relevant
authorities, and/or courts or tribunals in connection with the processing, handling, transfer and storage of
my personal data by the Employer or such third party as aforementioned.

By signing this consent form, I give my permission to the Company or any third party associated with it for
it/them to process my personal and sensitive data in accordance with the purpose/terms of this consent
form. I confirm I have carefully read and understood the consent form before completing it. I also confirm I
am aware of my rights under the Data Protection Act such as the right to access, rectify, and erase my
personal data and that I have a right to withdraw my consent at anytime such withdrawal to be in writing
and delivered to the Employer and duly acknowledged.
