Topic: Clinical Goals for clinical placement

My first goal for placement is to be able to build a therapeutic and professional relationship with patient
and staffs in the institution.

Objective

 learn how to communicate effectively and relate cordially with patients, family and staff
members in the facility.

Rationale

 To demonstrate good communication skills to enable patient express feelings freely and
understand any information I relay to them.
 Show empathy and respect the dignity of the individual using great oral language which will
enable me develop a great therapeutic relationship.

Strategies

 To be able to frequently use and understand medical terms in writing as well as educating
patient in certain situations which help in achieving effective communication and a therapeutic
relationship.
 To establish a rapport using good communication techniques
 To be able to handover and take up a patient’s care and properly document.

Evidence

This will be evidenced by nursing staff giving me a feedback on my attempt and progress in
communicating with them and clients during placement.

2. Integrating nursing skills and knowledge attained to provide safe and quality nursing care.

Objectives

 Ensuring confidentiality of patient’s information and providing privacy during


procedures at all times.
 Acknowledging the person’s right, values and cultural background.
 Ensuring infection control techniques

Rationale

 Practicing what I have studied will assist me in keeping a current knowledge base of
study and taking up ethical responsibilities.
Strategies

 Practicing proper hand hygiene at all times, thus, using the five moment of hand washing
throughout activities to prevent cross infection.
 Ensuring the use of personal protective equipment when necessary.
 Working in accordance with the rules and regulations of the institution and within my scope of
practice as a student.
 Confirming instructions before performing them and under supervision.

Evidence

Practicing these skills will enable me be confident in skills and be ready to perform certain procedures
perfectly.

3. Exhibiting proper and appropriate skills of observations of patients.

Objectives

 Know how to monitor the progress of patients using the basic skills of observation and charts.
 To know the abnormal from the usual so as to determine when to call for help. This is because
the patterns of the vital signs of a patient are included in the Early Warning Scoring (EWS) which
helps to initiate a quick response to a deteriorating condition, improve the safety and ensure
greater outcome.

Rationale

 This will assist in evaluating a patient’s progress towards a planned and expected outcome and
modify if the need be.

Strategies

 Familiarizing with the hospital equipment by observing, knowing how to use them and going
according to the protocol of the facility.
 Practice how to take vital signs and documenting under the supervision of the RN and then
continually do them to be perfect at it.
 Ensuring the use of therapeutic interaction to allay any anxiety by explaining procedure to
patients and for cooperation.

Evidence

Continuous practice of these will make me confident, skillful and able to determine when to call for
help.
4. Objective

 To know the proper ways of administering medications, the reason the client is taking that
medication, the usual dosage range, contraindication, its therapeutic and side effects.

Rationale

 This will help in evaluating the intended outcomes for the medication, and to be able to figure
out any error or question an order if unclear.

Strategies

 Note down any medication I come across for a patient and read about them, ask questions if
possible.
 I ensure that all the rights of medications are observed before administering any medication
(orals) and working within my scope of practice.
 Reporting any unusual findings for example with the dosages or any important information
missing

Evidence

This will take some time since there will be lots of medications to know about, however, with time I will
be able serve medications with confidence, and will be able to question any wrong order as I gain
experience.
