OUTCOMES
Typically surveys are used to evaluate outcomes at Hoke County
DSS in APS is:

Safety planning at intake and before exiting the program


An exit survey
In- Home Aide Client Satisfactory Survey
APPLY KNOWLEDGE OF HUMAN BEHAVIOR AND THE
SOCIAL ENVIRONMENT, PERSON-IN-ENVIRONMENT,
AND OTHER MULTIDISCIPLINARY THEORETICAL
FRAMEWORKS IN THE EVALUATION OF OUTCOMES.
A clients likelihood of gaining suitable housing is evaluated
As well as the likelihood of the client needing:

Safe place to live


Home care assistance
Home health nurse
Assisted living

A clients ability to obtain good nutrition is evaluated:

Does the client qualify for food stamps?


Are there food banks in their area of residence?
Do they have transportation to get to food banks and DSS?
CRITICALLY ANALYZE, MONITOR, AND
EVALUATE INTERVENTION AND
PROGRAM PROCESSES AND OUTCOMES

THE MICRO, MEZZO, AND MACRO LEVELS
The evaluation findings help social workers with Adult Protective
Services determine:

If more action is needed to find additional housing, assisted living facilities


and/or apartments for the elderly
If cases are being handled properly and in an efficient amount of time
Making sure best interests of the client are being met
