HOW TO COMPLETE AN
OARS REPORT

2) Select the campus the incident occurred and then click Select to specify where
the incident occurred. You can search for the location by leaving the fields blank
or entering the whole or partial name of the building, address, parking lot, etc.
Once found, click Select Location.
3) Give a detailed description of the incident; the more information, the better. You
can also attach supplemental materials on this page, such as photos.
4) Once Page 2 is complete, click Next in the bottom left corner of the page.
2) Once the corrective actions are completed, enter the complete date. Indicate
that the actions are complete by selecting Yes.
The log may also assist employers in capturing information on how workers are hurt and
identifying health and safety hazards in the effort to prevent future injuries or illnesses.
Step 2
Click on Open OSHA 300 Form.
Step 3
Choose the OARS report for which you
wish to complete the OSHA 300
form. Click Open OSHA 300
Form.
