What is the common theme in these accidents
• There are 2 sides to every accidents; technical and
behaviour
• Technical issues (e.g. design) can be solved scientifically
• However, failing to effectively change the behavioural
(human factor /error) will make us vulnerable to another
major incident.
What is human factor?

 Human factors evaluate the interaction between the

people, workplace & management system

 The science of human factors focuses on these

elements
Why use human factor?

 When accidents happen due to human error, it is

important to see what goes behind the error.

 Find out how the human (worker) actions were

influenced by different elements in the workplace.

 Human factor approach focus on these elements. Helps

to see a bigger picture.
