• INTRODUCTION:
• Clinical trails are conducted only when
• satisfactory information has been gathered on the quality of the nonclinical
safety
• Health authority/ethics committee approval is granted in the country where
approval of drug is sought
• Clinical trail is the mainstay for bringing out new drugs to the market .
• Drug review
• Before one can initiate testing in human begins, extensive pre-clinical or
laboratory research is required.
• Research usually involves years of experiments in animal or human cells
• If this stage of testing is successful, i.e.., phase 0 or micro dosing studies
then the sponsor provides the data to the FDA requesting approval to
begin testing in humans . This is called Investigational New Drug
Application( IND)
Schedule Y
Requirements and guidelines for permission to import and/or
manufacture of new drugs for sale or to undertake clinical trials.
Purpose of Schedule Y
• To frame guidelines for conduct of clinical research
• Control and regulation for new drugs
• CDSCO and Drug Technical Advisory Board formulated
GCP under schedule Y in 2005
Why India ?
• Large patient pool
• Well trained and experienced Investigators.
• Considerable low per patient trial cost, as compared to
developed countries.

Clinical Trial
2.Responsibilities of Sponsor
• Implementing and maintaining Quality Assurance systems.
• Submit status report to the Licensing Authority periodically.
• Any unexpected Serious Adverse Event (SAE) should be reported to the
licensing authority within 14 calendar days.
3.Responsibilities of Investigator
• To ensure that adequate medical care is provided to the subject.
• Any Serious adverse events (SAE) and unexpected Adverse Event (AE)
should be reported to the sponsor within 24hours and to the EC within 7
working days.
4.Informed Consent
• In all trials, a freely given informed written consent is required to be
obtained from each study subject.
• The Investigator must provide information about the study verbally as
well as using a Patient Information Sheet in a Non-technical and
understandable language to the subject.
5.Responsibilities of Ethics Committee
• To review and accord its approval to a Trial protocol to safeguard rights,
safety and well-being of all trial subject and to protect RSW of all
vulnerable subjects
• Should conduct ongoing review of the trials, at appropriate intervals.
• These studies are performed after drug approval and related to the
approved indication.
• These studies includes drug-drug interaction, dose-response or safety
studies, mortality/morbidity studies, epidemiological studies, etc.
3.STUDIES IN SPECIAL POPULATION
• Geriatrics
• Pediatrics
• Pregnant or Nursing Women
• Post Marketing Surveillance
– Closely monitored new drugs clinical safety
– Periodic Safety Update Report (PSUR)- to report all relevant new
information
– PSUR shall be submitted every 6 months for the first 2 years
• Special Studies-BA/BE studies
– Conducted according to the guidelines for BA and BE studies.
– Evaluation of the effect of food on absorption following oral
administration

Study synopsis : This section will only summarize the important conclusions
derived from the study.

Ethics committee : A declaration should state that EC notifications as per Good
Clinical Practice Guidelines issued by Central Drugs Standard Control Organization
and Ethical Guidelines for Biomedical Research on Human Subjects, issued by
Indian Council of Medical Research have been followed.
Study team : administrative structure of the study (investigators site staff ,
sponsors)
Introduction : description of product development
Study objective : overall purpose of the study and the primary and secondary objectives
to be achieved should be mentioned
Investigational plan : the overall trial design, the Subject selection criteria, the
treatment procedures, blinding / randomization techniques if any, allowed! Disallowed
concomitant treatment, the efficacy and .safety criteria assessed, the data quality
assurance procedures and the statistical methods planned for the analysis of the data
obtained.
Trial subjects: a clear accounting of all trial subjects and all the cases that were dropouts
or protocol deviations must be mentioned
Efficacy evaluation : results of evaluation of all efficacy variables with proper tabular
and graphical representations
Safety evaluation : (A) all serious adverse events, whether expected or unexpected (B)
unexpected adverse events whether serious or not (compiled from data received as per
Appendix XI). The comparison of adverse events across study groups may be presented
in a tabular or graphical form, This section should also give a brief narrative of all
important. Events considered related to the investigational product.
